Resident Care Manager Nurse (RCM)

About the role

Granite Rehabilitation and Wellness is currently looking for a Resident Care Manager Nurse (RCM) to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversees the work of the Licensed Nurses and CNAs in our facility.

Responsibilities

• Obtain and administer prescribed medications and treatments in accordance with approved nursing techniques.
• Observe residents, record significant conditions and reactions, and notify supervisor, family (responsible party), and physician of changes in condition, reactions to drugs, treatments, and significant incidents.
• Take vital signs to detect deviations from normal and assess condition of residents.
• Administer appropriate disciplinary action for nursing staff non‑compliance with state and federal regulations and Center policies.
• Modify resident care plan, care directives, Activities of Daily Living (ADL) and aide flow sheets in conjunction with resident changes in condition.

Requirements

• Graduate of an accredited school of nursing.
• Current RN or LPN licensure in the state of employment.
• Demonstrated knowledge of the nursing process, particularly as it relates to geriatric residents, long‑term care, and rehabilitation.
