Resident Care Manager/LVN

About the role

The Resident Care Manager/Nurse at BridgePoint at Los Altos directs, coordinates, and supervises care services provided by care associates for the residents within the Assisted Living Department. They ensure exemplary service is delivered with integrity, dignity, and compassion. They may also be responsible for medication control and coordination with physicians.

Responsibilities

• Makes rounds to monitor and evaluate assigned residents and regularly assess levels of care, whenever there is a change in condition and/or to determine whether a resident is no longer appropriate for the setting, notifying Assistant Living Director as required
• Aids residents with simple requests as needed or finds appropriate associates to assist
• Ensures residents' physical, social, cultural, psychosocial, and intellectual well-being
• Supervises departmental operations as directed on a day-to-day basis under the guidance of the department director or Executive Director
• Responds tactfully and promptly to all residents, family members, and/or staff
• Monitors resident medication programs and ensures all physician orders are followed accurately
• Interfaces with physicians, care providers, and outside health services regarding the resident's needs
• Reviews resident reports and documentation and notifies the Assisted Living Director of any changes in a resident's condition or needs
• Assesses residents using the company's resident care software system
• Manages or assists with any emergency situations responsibly and calmly until aid arrives
• Assists with scheduling for Care associates or care conferences
• Plans, prepares, and monitors daily work schedules to meet labor targets
• Manages direct reports' schedules and time-cards as necessary
• Reviews payroll documentation to minimize missed punches, monitor overtime, and manage labor costs
• Communicates the company vision and strategy to create alignment between daily activities and company strategy

• Plays a key role in planning and preparing assignment sheets for Resident Assistants
• Maintains high standards of personal appearance and grooming, which includes wearing the proper uniform and name tag when working
• Interviews, hires, trains, manages, and retains highly effective associates and creates a culture of associate engagement
• Coaches, mentors, teams-builds, performs performance management, recognizes, and develops associates
• Identifies and develops top talent for future succession opportunities
• Takes a proactive approach to increase efficiency and potentially reduce costs to support continuous improvement
• Ensures regulatory compliance and reports any issues or concerns immediately to the Assisted Living Director and Executive Director
• Supports and assists as needed in the community sales process

Requirements

• Previous supervisory experience
• High school graduate required; college course work preferred
• Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) is preferred
• One to three years’ experience working in senior care
• Special Certifications/Requirements: Must have or be able to obtain the appropriate State license required by law if required, CPR Certified preferred, Basic competency in the use of multiple forms of technology, including Microsoft Office applications, internet-based and custom, Ability to work a flexible schedule, including weekends and holidays as needed
