Resident Care Manager (LPN)

Job Summary

Alaska Gardens Health and Rehabilitation Center is seeking a Resident Care Manager (LPN) to provide direct nursing care to residents and supervise the day-to-day activities performed by nursing assistants. The role involves developing a plan of care for residents, arranging and coordinating all services, and ensuring residents' health, safety, and general wellbeing.

Essential Functions

• Supervise nursing care to residents.
• Comply with state and Federal Regulations.
• Develop and review resident’s medical and nursing treatments to ensure they align with the resident’s care plan and wishes.
• Plan, schedule, and revise the MDS, including implementing RAPs and Triggers.
• Make daily rounds to ensure nursing personnel are performing their work assignments.
• Review nurses’ notes to ensure they are informative and descriptive of the nursing care provided and the resident’s response to care.
• Participate in reviews of discharge plans and prepare reports for the Care Plan Committee as directed.
• Delegate, train, evaluate, and support RN, LPN, and CNA personnel.
• Provide direct nursing care as necessary and assist the Director of Nursing as needed.
• Make recommendations for new or changes in procedures, policies, methods, education, reference material, and general nursing practices to ensure high-quality patient care.

Education, Licensure, & Experience Requirements

• Graduate of an accredited school of nursing.
• Current WA RN licensure in good standing.
• Minimum 2+ years in supervisory/management position, including experience with budgets, staff development, training, and scheduling.
• Minimum 1 year of experience as a supervisor in a healthcare setting.
• Training in rehabilitative and restorative nursing practices.

Knowledge, Skills & Abilities

• Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.
• Working knowledge of MDS process, state and federal surveys, and CMS.
• Experience with Electronic Medical Records and computer documentation systems.
