About the Role

As a Full-Time Resident Care Coordinator at Everbloom Memory Care, you will play a vital role in overseeing the daily care and wellbeing of our residents. This onsite role offers a unique opportunity to cultivate meaningful relationships and provide compassionate care in a supportive environment. Responsibilities

• Coordinate individualized care plans, ensuring the highest standards of personalized assistance.
• Conduct regular assessments of residents' health and emotional needs.
• Facilitate communication between families and care staff.
• Implement activities that promote engagement and social interaction.
• Lead training sessions for new staff, fostering a culture of excellence and empathy.
• Maintain meticulous documentation of care services.
• Participate in interdisciplinary team meetings to enhance care strategies.
• Assist in managing the community schedule (on-call position).

Requirements

• At least two years of supervisory or managerial experience, demonstrating progressive responsibility in a caregiving environment.
• Solid understanding of nursing practices, relevant laws, regulations, and guidelines related to residential care facilities.
• Familiarity with Electronic Medical Records and computer documentation systems.
• Experience in current federal and state healthcare regulations.
• Proficiency in Microsoft 365 applications, including SharePoint, Teams, Word, Excel, and Outlook.
• Excellent writing and interpersonal skills for collaborating with residents, staff, and management.
