Responsibilities
- Managing patient census
- Closing care gaps
- Coordinating community resources
- Providing patient education on chronic conditions
- Managing medications
- Facilitating timely services such as home health and DME
- Resolving patient inquiries
- Managing referrals and appointments
Requirements
- Active multi-state or compact LPN license
- High-speed Wi-Fi
- HIPAA-compliant workspace
- Physical ability to sit for long periods and communicate clearly
Qualifications & Skills
- Technical proficiency in Microsoft Office
- Excellent communication skills
- Preferred: Epic experience
- Preferred: Bilingual abilities
