Remote - Licensed Practical Nurse (LPN/LVN) - Day Shift

About the Role

Seeking an experienced Licensed Practical Nurse (LPN) to provide comprehensive remote care management for patients, focusing on value-based care initiatives.

Responsibilities

• Manage patient caseload to close care gaps through a resolution-driven approach.
• Coordinate community resources and provide education on chronic condition management.
• Perform medication management, including reconciliation and adherence.
• Ensure timely delivery of services like Home Health, DME, and Home Infusion.
• Assist with referrals, appointment scheduling, and address patient questions.

Requirements

• Active multi-state or compact LPN license.
• Minimum of 3 years of patient-facing experience.
• High-speed Wi-Fi and a HIPAA-compliant workspace.
• Position involves sedentary work, requiring long periods of sitting, clear communication, and attentive listening.

Skills

• Technical proficiency in Microsoft Office.
• Excellent communication skills, both written and verbal.
• Experience with Epic and bilingual skills are a plus.
