Remote Licensed Practical Nurse | Licensed Vocational Nurse Patient Advocate (Contract/100% Remote)

About the role

Solace is a Series C healthcare startup that pairs patients with expert advocates to navigate the complex U.S. health system. The Patient Advocate role is a 1099 independent contractor position supporting Medicare members remotely. You will work with patients via video or phone, helping them untangle complicated health situations, move toward wellness goals, and address social determinants of health. The role offers flexible scheduling — full‑time, part‑time, or anywhere in between — and is fully remote.

Responsibilities

• Lean on your LPN/LVN clinical background while becoming fluent in Solace’s proprietary platform, tools, and standards.
• Turn empathy and attentive listening into patient relationships built on genuine trust.
• Read each patient’s situation, prioritize what matters most, and shape a care plan that keeps things moving over time.
• Design care plans that go beyond the clinical, factoring in everyday barriers such as access to food, transportation to appointments, and home support.

Requirements

• Current LPN or LVN license in good standing.
• Proven experience working with patients from diverse backgrounds and communities.
• Compassion rooted in caring for people and advocating for those who cannot advocate for themselves.
• Steadiness and structure: ability to manage multiple priorities and stay organized when the pace picks up.
• Quick technical footing: comfortable learning unfamiliar systems quickly.
• Ownership mindset: spotting what needs handling and taking initiative without waiting for direction.
• Based in the United States.
