About the role

We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey. The role involves leading clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends.

Responsibilities

  • Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with our team of RNs and MDs.
  • Manage the patient's treatment plan and medications with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for Hypertension and Type 2 diabetes.
  • Respond to patient escalations identified via RPM including abnormal vitals, symptoms, and labs.
  • Lead regular virtual check-ins with patients to review labs and GDMT optimization and adherence.
  • Ensure every healthcare interaction with Cadence is an exceptional experience that prioritizes the well-being of the patient and aligns with the goals of the health system.
  • Support internal growth efforts to help Cadence scale exceptional care delivery to patients with CHF and other chronic conditions including Hypertension and T2D.
  • Be instrumental in shaping the culture of one of the fastest-growing teams at Cadence.

Requirements

  • Licensure Requirements: Required - Active Compact Multi-State RN license. Required - Active NP licensure in at least two of the following states: TX, MI, NC, CA, PA, NJ, CT, AL. Exceptional candidates with fewer licenses may be considered based on overall qualifications and ability to obtain additional priority state licenses. Licenses must be active and in good standing at time of hire.
  • Schedule Requirements: Monday through Friday schedule (Flexible scheduling available, with a core shift schedule of 9:00 AM to 6:00 PM in Mountain, or Pacific Time).

Qualifications

  • 5+ years experience treating patients with chronic diseases (CHF, Hypertension, T2D) either in an outpatient or inpatient setting at a high-performing medical center.
  • Experience managing CHF with GDMT.
  • Active ANCC or AANP certification.
  • Master’s Degree as a Nurse Practitioner with the willingness to expand state licensure as Cadence adds new markets and partners.

  • Experience working in a CHF bridge clinic or T2D clinic, a startup environment, and working with remote patient monitoring technology is preferred.
  • Comfortable using AI-assisted clinical tools (such as Gemini, Open Evidence, or comparable platforms) while maintaining clinical judgment as the primary guide for every patient decision.
