Responsibilities
- Evaluate and assess patients’ physical therapy needs
- Develop individualized treatment plans
- Provide therapeutic exercises and rehabilitation services
- Educate patients and families on treatment plans and mobility improvement
- Cookise care with physicians and interdisciplinary team members
- Complete timely and accurate documentation
Qualifications
- Active Colorado Physical Therapist (PT) license
- Home Health experience preferred, but not required
- Strong communication and patient care skills
- Reliable transportation preferred
