About the role

We are seeking a compassionate and motivated Physical Therapist to join our growing team. In this role, you will:

  • Evaluate residents and develop individualized plans of care.
  • Provide skilled physical therapy treatments, including therapeutic exercise, gait training, balance training, transfer training, and functional mobility interventions.
  • Reassess patient progress and modify treatment plans as appropriate.
  • Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
  • Complete timely and accurate documentation using the electronic medical record (EMR).
  • Educate residents, caregivers, and facility staff on treatment plans, home exercise programs, fall prevention, and safe mobility techniques.
  • Collaborate with interdisciplinary team members to provide coordinated, patient-centered care.
  • Maintain a productive clinical caseload while delivering exceptional therapy services.

Requirements

Qualifications:

  • A current State Physical Therapist (PT) License.
  • A current CPR/BLS Certification.
  • A valid driver's license, reliable transportation, and auto insurance.
  • Experience in geriatrics, home health, assisted living, or skilled nursing is preferred but not required.

Per-Visit or Salary (based on years of experience).

Schedule

Monday-Friday ⎸Full-Time (Weekends available but not required).
