Job Requirements

  • Certification as a CNA or a bachelor’s degree from an accredited university or institution of higher learning (preferred)
  • High School Diploma or G.E.D with two years of experience, or one year as an HNNR Rehab. Tech. and have completed all HNNR Core and Advanced Medical Rehab Tech Skills requirements
  • One year experience as a Hope Network Rehabilitation Technician or two years of experience in a human service agency
  • Work experience in a licensed residential setting preferred
  • Valid driver’s license
  • Able to lift 50 pounds
  • Able to become certified in CPR/First Aid
  • Commitment to scheduled shifts, assigned duties, and performance expectations
  • Basic computer skills

Responsibilities

  • Serves in the training of newly hired staff
  • Assists with documentation for various business purposes
  • Specializes in specific healthcare needs as assigned by program manager
  • Maintains safety 24/7, which may involve eyes-on, one-on-one, 15- or 30-minute checks, etc.
  • Prevents, intervenes, and resolves crises
  • Establishes collaborative relationships based on trust and empathy through consistent and meaningful communication
  • Fosters partnerships with clients, the community, and other professionals
  • Helps build effective skills for problem-solving, communication, and independence
  • Assists clients in identifying and/or participating in social and leisure interests
  • Supports clients with Activities of Daily Living, including but not limited to getting to and from places, cooking, dressing and undressing, bathing and showering, combing hair, trimming nails, restroom assistance
  • Aids clients with bowel and bladder programs, feeding (G-tubes), wound care, and transfers
  • Maintains vitals for any medical conditions or illnesses
  • Promotes positive behavior
  • Attends medical appointments and ensures accurate information is relayed between medical provider and care team
  • Understands, reads, and assists in the implementation of medical, behavioral, and/or clinical team care plans and provides accurate, timely, and descriptive documentation of such
  • Oversees client medication needs
  • Connects clients with opportunities in their community to develop relationships with others
  • Supports employment, educational, and volunteer interests and goals
