Job Summary And Qualifications Provide total adult and geriatric physical rehabilitation nursing care through a multi-disciplinary process resulting in the attainment of optimum quality of life for the patients. Major Responsibilities: Note: The following is a list of the essential functions of the job. Most positions can be described in 6-8 major responsibility areas. Accountabilities Assessments of patients initially at admission and throughout the stay. Participation in plan of care and implementation with multi-disciplinary team. Documentation of progress, goals, treatments, education throughout stay. Patient care – Delivers and documents direct quality patient care specific to Rehab Standards of Care. Prioritize/achieve patient goals/needs through analysis of assessments, shift reports, diagnostic test results, and other sources of info to recognize potential/actual patient problems, and respond appropriately. Performance improvement – Participates in improvement of care delivery through unit based performance improvement committees and activities. Identifiable – Information is identified but further investigation often is required in order to define problems and proceed. Often requires decision new methods and/or modifying existing procedures/precedents within established policy. Medication – Administers medications and intravenous fluids accurately and safely. Recognizes and reports medication events promptly with appropriate intervention when needed. Environment of Care – Provides a safe environment for patients, staff, and visitors by understanding and abiding by the Infection Control, Environment of Care policies as well as the OSHA and JCAHO standards. Leadership =- Demonstrates leadership through delegation of assignments to team members based on competency, skill level, and patient acuity. Participating in staff competency and advancement through involvement in Preceptor Program. Education – Actively seeks best practices to enhance clinical skills by attending seminars, classes, annual updates, staff meetings, etc. Provides supports by: Promoting consistent positive patient interactions that advance the agenda of unparalleled patient service Practicing and adhering to the “Code of Conduct” philosophy and “Mission and Value Statement” Performing other duties as assigned Education & Experience: Associate's degree Nursing or RN Diploma Required 1+ year(s) of experience in Physical Rehabilitation Required Or equivalent combination of education and/or experience Licenses, Certifications, & Training: Registered Nurse - Currently licensed as a registered professional nurse in the state in which he or she practices, in accordance with law and regulation.
