About the role
Provide holistic, evidence-based rehabilitative nursing care to adult patients in a faith-inspired clinic.
Responsibilities
- Perform comprehensive assessments and develop individualized care plans.
- Coordinate closely with PT, OT, and SLP teams to support patient recovery.
- Educate patients and families to facilitate safe transitions and functional improvements.
- Document care activities in the EMR and collaborate with an interdisciplinary team.
- Uphold infection control and quality standards while promoting health equity.
