Rehab Patient Care Coordinator - (PT/OT/SLP)

What You'll Do:

• Cook up and manage patient care services
• Act as a go-between for patients, families, providers, and caregivers
• Handle referrals, authorizations, scheduling, and care plan coordination
• Participate in interdisciplinary case conferences
• Keep track of patient progress and ensure smooth care transitions
• Support quality assurance, chart audits, and regulatory compliance
• Collaborate with clinical and administrative teams to enhance patient satisfaction and outcomes

Qualifications:

• Active California PT, OT, or SLP License
• Current BLS Certification
• Home Health experience preferred
• Knowledge of Medicare regulations and OASIS preferred
• Strong organizational, communication, and computer skills
• Leadership, care coordination, or management experience is a plus
