About the Role
Provide evidence-based rehabilitative care to adult patients.
Responsibilities
- Assess patients and develop individualized care plans.
- Coordinate with an interdisciplinary rehab team to support patient mobility, activities of daily living (ADLs), and safe discharges.
- Document care accurately in the Electronic Health Record (EHR).
- Educate patients and their families about treatment plans and health management.
- Promote quality and safety in patient care.
Requirements
- Current RN license.
- Compassionate with strong assessment and communication skills.
