Rehab Nurse

Role: Provides comprehensive nursing care to residents, including assessment, planning, intervention, and documentation of health status and needs. Responsibilities: Assess residents' health, educational, and environmental needs upon admission and continuously monitor changes.Develop, review, and revise individualized care plans collaboratively with the interdisciplinary team, residents, and families.Implement nursing interventions such as medication administration, treatments, and IV therapy safely and accurately.Communicate effectively with team members and families, utilizing tools like SBARR for timely physician notifications.Monitor residents' conditions closely, identify problems, and utilize QAPI processes for immediate resolution.Assist with resident care activities, including ADLs and responding to call lights.Maintain compliance with regulatory requirements and documentation standards.Participate in ongoing education to stay current with nursing best practices. Qualifications & Skills: Registered nurse license in the applicable state.Current CPR/AED certification or willingness to obtain within six months.Critical thinking, excellent communication, time management, and organizational skills.Physical ability to stand, walk, lift, and assist residents and equipment safely.Manual dexterity and visual acuity for accurate documentation and patient care.Preferred experience in long-term care setting.
