Rehab Liaison

About the Role

As our Rehab Liaison, you will coordinate the acute rehabilitation admission process, including pre-admit screening, financial authorization, and communication with the program medical director and rehab director. You will actively engage with existing referral sources to nurture relationships while identifying and cultivating new clients to expand our referral base. A critical part of your role involves consulting with physicians to complete appropriate pre-screening documentation, ensuring all necessary information is gathered for patient admissions.

To succeed, you will need clinical experience, a strong understanding of rehabilitation services, exceptional interpersonal and communication skills, and the ability to build relationships with referral partners and physicians. A proactive approach to business development, meticulous attention to detail in documentation, and a passion for advocating for patients in need of rehabilitation are essential to maintaining and growing our inpatient population.

Responsibilities

• Coordinate the acute rehabilitation admission process, including pre-admit screening and financial authorization.
• Communicate with the program medical director and rehab director to facilitate patient admissions.
• Engage with existing referral sources to nurture relationships and cultivate new clients.
• Consult with physicians to complete pre-screening documentation.
• Advocate for patients requiring rehabilitation services.

Requirements

• Minimum of five (5) years of clinical experience.
• Current California licensure as a Registered Nurse (RN), Physical Therapist, Occupational Therapist, or Speech-Language Pathologist (SLP).
• Bachelor’s degree (Master’s degree preferred).
• One year of clinical liaison experience preferred.

Pay

$37.50 - $51.45 per hour.
