About the role

Direct all activities of the Rehabilitation Department, including clinical assessments, record keeping, compliance with treatment plans, case management, and state licensure requirements. Provide quality resident care to enable residents to achieve and maintain maximum functional ability while fostering positive interactions.

Responsibilities

• Coordinate all activities and services provided by the rehabilitation department.
• Assist in coordinating admissions of residents requiring skilled rehab services.
• Evaluate, develop, organize, and implement resident care programs through individual and group treatment sessions.
• Determine and schedule staff daily based on patient needs.
• Coordinate rehab requirements and the plan of care for each resident.
• Conduct patient referral marketing in conjunction with admissions programs.
• Ensure completion of required quality assurance and program evaluation activities.
• Complete HR functions including interviewing candidates, posting job openings, conducting performance/competency reviews, monitoring time sheets, and managing progressive discipline.
• Manage a personal caseload, identifying residents through screening and scheduling treatments individually or in groups while maintaining efficiency.
• Instruct residents, families, and caregivers on post-discharge treatment continuation.
• Ensure all documentation and treatment conform to billing policies and procedures.
• Provide oversight and direction to rehabilitation personnel and assist in supervising and educating affiliating students.
• Maintain sanitary, orderly, and safe physical surroundings and equipment.
• Comply with ethical, regulatory, procedural, and professional standards and practice guidelines.
• Maintain resident confidentiality and privacy.
• Foster residents’ self-worth and respect by treating them with kindness and understanding.
• Know and comply with the Residents’ Bill of Rights.
• Report grievances, complaints, and incidents involving rehab staff immediately.
• Maintain professional competence and stay updated on creative trends in rehab treatment through continuing education, seminars, and training programs.
• Attend all mandatory in-services.

Requirements

• Graduate of an APTA-accredited school and eligible for licensure.
• Prior long-term care experience preferred.
• Good physical and mental health.
• Frequent standing and walking; may require turning, stooping, bending, stretching, crouching, fine hand/eye coordination, and lifting during treatments.
• Visual and hearing acuity to detect changes in residents’ conditions and respond to pages.
• Ability to communicate effectively with residents, families, staff, co-workers, medical personnel, regulatory agencies, and consultants.
