Responsibilities
- Provide skilled physical therapy services according to established plans of care.
- Deliver therapeutic exercise, gait training, balance activities, transfer training, and mobility interventions.
- Monitor patient progress and modify treatment plans as needed to achieve functional goals.
- Supervise Physical Therapist Assistants (PTAs) in accordance with state regulations.
- Complete timely and accurate documentation using the electronic medical record (EMR).
- Educate residents and caregivers on home exercise programs, fall prevention, and safety strategies.
- Collaborate with interdisciplinary team members to provide coordinated, patient‑centered care.
- Maintain a productive clinical caseload while delivering compassionate, high‑quality therapy services.
