About the role

Greenfield Rehab is seeking an experienced and dedicated Physical Therapist (PT) to serve as our Rehab Director and lead therapy services in our skilled nursing facility in Green Bay, WI.

Responsibilities

• Provide overall leadership, coordination, and management of the rehabilitation department within a skilled nursing facility.
• Provide clinical and administrative oversight of physical, occupational, and speech therapy services.
• Maintain high standards of patient care, regulatory compliance, and operational efficiency.
• Function as a licensed Physical Therapist, delivering direct patient care as needed.

Qualifications

• Graduate from an accredited school with a minimum of a bachelor's degree in physical therapy.
• Licensed by the State of Wisconsin to practice as a Physical Therapist.
• Minimum 1 year of experience as a treating clinician.

• Strong clinical reasoning and patient identification skills.
• Experience managing medical complexity and changing functional status.
