Rehab Director - COTA

About the role

As a Rehab Director with Legacy Healthcare Services, you serve as the primary Legacy representative at the facility, promoting Legacy policies, clinical programs and supporting the mission to help aging adults live safe, healthy lives full of strength, movement, and joy. You will build relationships with patients, learn new ways to deliver care, and work with a team dedicated to helping senior patients live their best lives.

Responsibilities

• Create rapport and positive working relationships with key facility personnel and fellow Legacy employees to discover others’ needs.
• Oversee internal marketing efforts to assist with relationship building with staff and residents while keeping community leaders apprised of Legacy’s programs and other offerings.
• Manage site productivity and make necessary changes to achieve budget goals.
• Maintain a caseload that assists the rehab department in meeting standards.
• Assess patient needs and develop clinical programs appropriate for the facility census.
• Direct and train others in clinical service delivery across disciplines.
• Coordinate daily rehab department activities with facility staff to plan and implement therapy programming.
• Participate in clinical outcomes reporting and share results with the customer.
• Manage staff schedules to meet caseload requirements and utilize scheduling tools for patient appointments.
• Manage CliniFax and ensure all physician signatures are obtained on therapy plans of care.
• Create and maintain team relationships and open communication among rehab staff, facility staff, and Legacy corporate personnel.
• Assist with orientation and training for staff, guided by ARD/MSM, to meet company, state, and federal standards.

Requirements

• Clinical background as a Physical Therapist (PT), Physical Therapy Assistant (PTA), Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Speech‑Language Pathologist (SLP).
• Three to five years of experience in long‑term care (LTC) or senior living.
• Some prior supervisory experience preferred but not required.
• Minimum of a degree from an accredited college or university or equivalent experience (e.g., degree in PT, PTA, OT, COTA, SLP).
• Licensed and registered (PT, PTA, OT, COTA, SLP) in the state where assigned.

Schedule

Desirable Monday‑Friday schedule with no weekend or holiday requirements.
