Rehab - Clinical Nurse

Role Overview

This position involves providing direct patient care in a 9‑bed, open 24‑hour unit, collaborating with physical therapy, occupational therapy, and speech therapy professionals. The role focuses on assessment, diagnosis, planning, implementation, and evaluation within nursing standards to promote patient safety, quality, and family‑centered care. Responsibilities include educating patients and families on managing illness, developing and documenting personalized care plans, and assessing physical, psychosocial, emotional, cultural, and spiritual needs. The position requires working in a safety‑sensitive environment with alertness and adherence to safety protocols.

Key Duties

• Promote patient and family‑centered care, including education on post‑treatment needs
• Participate in safety, quality, and regulatory activities
• Develop and communicate individualized care plans with the care team and patients
• Assess patient needs holistically, respecting their values and barriers to involvement

Qualifications & Skills

• Minimum of a Nursing Diploma or Associate's degree in Nursing
• Current RN licensure required
• No prior experience necessary; two years of experience preferred for advanced roles
• Excellent communication and critical thinking skills
• Ability to collaborate across interdisciplinary teams
• Commitment to ongoing professional development and safety
