Rehab Clin Case Manager PRN

About the Role

The Rehab Clinical Case Manager assesses patients' needs upon admission and assists with the coordination of interdisciplinary treatment plans based on CMG, estimated length of stay, and functional efficiency goals to ensure achievement of predicted outcomes. This role also involves monitoring and updating the interdisciplinary care plans as appropriate.

Requirements

• Diploma or Associate of Science in Nursing (ASN) with graduation from an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
• Minimum of one (1) year of Utilization Review (UR) or Case Management experience in a hospital setting.
• Minimum of three (3) years of related clinical practice in an acute care or inpatient rehab setting.
• Previous experience with hospital information systems (order entry, results reporting, case management).
• Proficient experience using commonly used computer applications (e.g., word processing).
• Active Florida Registered Nurse (RN) License.

Preferred Qualifications

• Active Case Management Certification (CCM or ACM) or certification within one (1) year of hire.
• Membership in a relevant Case Management organization.
• Demonstrated ability to manage multiple tasks and adjust priorities according to patient and department/hospital needs.
• Solid clinical assessment, critical thinking, decision-making, and organizational skills.
• Strong verbal and written communication skills.
• Strong interpersonal skills and ability to work collaboratively with leaders, staff, patients, families, healthcare teams, payers, and external agencies.
