Rehab Clin Case Manager PRN

About the role

The Rehab Clinical Case Manager assesses the patients' needs upon admission and assists with the coordination of interdisciplinary treatment plans based on CMG, estimated length of stay, and functional efficiency goals to ensure achievement of predicted outcomes, as well as monitoring and updating the interdisciplinary care plans as appropriate.

Responsibilities

• Assess patients' needs upon admission and coordinate interdisciplinary treatment plans
• Ensure achievement of predicted outcomes through monitoring and updating care plans
• Collaborate with interdisciplinary teams to develop and implement treatment plans
• Manage patient information systems and utilize computer applications for documentation
• Communicate effectively with patients, families, healthcare team, payers, and external agencies

Requirements

• Diploma or Associate of Science in Nursing (ASN) with graduation from an accredited school of nursing
• Bachelor of Science in Nursing (BSN) preferred
• Minimum of one (1) year of Utilization Review (UR) or Case Management experience in a hospital setting
• Minimum of three (3) years of related clinical practice in an acute care or inpatient rehab setting
• Previous experience with hospital information systems (order entry, results reporting, case management)
• Proficient experience using commonly used computer applications (word processing a minimum)

Preferred Qualifications

• Active Case Management Certification (CCM or ACM) or within one (1) year of hire
• Membership in relevant CM organization
• Demonstrated ability to manage multiple tasks and adjust priorities according to patient needs and department/hospital needs
• Solid clinical assessment, critical thinking, decision making and organizational skills
• Strong ability to communicate effectively both verbally and in written work
• Strong interpersonal skills and ability to work collaboratively with leaders, staff, patients, families, healthcare team, payers and external agencies
