About the Role
Provides care for a specific patient population with various types of chronic wounds and their advanced wound care treatments. Serves as a resource for other health care professionals, patients, families, and the community.
Responsibilities
- Performs assessment and reassessment of the physical and educational needs of patients with chronic wounds needing advanced wound care techniques under the guidance of a physician and/or a Certified Wound Care Clinician.
- Communicates patient needs to appropriate members of the wound care team and provides discharge teaching as required.
- Assists with referrals for studies, labs, procedures, and other physicians.
- Formulates, implements, and evaluates an individualized plan of care based on patient needs through consultation and collaboration with the wound care team.
- Coordinates care in an efficient, cost-effective manner using knowledge of current advanced wound care modalities and practices.
- Assists with education of patients and caregivers on current evidence-based advanced wound care modalities and practices.
- Promotes knowledge of healthy practices in the area of expertise throughout the community through education.
- Identifies actual or potential issues of resource management and patient care; works with the wound care team to identify cost-effective alternatives and facilitates discharge.
- Keeps documentation via EMR current, including accurate documentation of products and facility charges.
- Maintains a safe, comfortable, clean, and therapeutic environment for patients and families per established departmental policies.
- May perform hyperbaric oxygen therapy-related duties after completing the required hyperbaric training.
- Performs other duties as assigned.
Requirements
- Associate degree in Nursing; Bachelor's degree in Nursing preferred.
- Two (2) years of nursing experience, with experience in skin and wound care.
- Wound Vac experience preferred.
- Holds a current RN compact/multistate license recognized by the NCSBN Compact State or is licensed to practice as an RN in the state the team member is working.
- Certification in Wound and Ostomy preferred.
