About the role

The RN will provide a full range of skilled nursing care to patients in a homecare setting with a focus on patient education and palliative care to meet the physical and emotional needs of patients, while educating and providing support to families.

Responsibilities

  • Provide skilled nursing care to patients in a homecare setting
  • Collaborate with interdisciplinary team to support patients to remain in the setting of their choice
  • Educate and provide support to families
  • Focus on patient education and palliative care

Requirements

  • Associate's degree in nursing or higher
  • Minimum of one or more years of directly related home or community hospice nursing experience
  • OR, two or more years of related home or public health nursing experience
  • OR, three or more years of general nursing experience, including at least one year of directly related home/public health or hospice nursing experience
  • Certification in hospice and palliative care preferred

Qualifications

  • Current RN licensure recognized by the State of Vermont required
  • Appropriate experience in specific clinical area. Varies by unit

Skills

  • Strong communication skills
  • Ability to work independently and as part of a team
  • Knowledge of hospice and palliative care principles

Pay

  • $10,000 Sign On Bonus eligible for full time external applicants

Schedule

  • Minimum of 20 hours a month after orientation
  • Availability on weekends and overnights
