Position Summary

The Hospice Registered Nurse acts as the Case Manager for their hospice clients. They work closely with all the members of the Interdisciplinary Team (IDG) to create a highly personal and individualized plan of care for their hospice clients. They are responsible for organizing and directing the services for hospice clients, their families and caregivers based on identified needs and client/ family goals.

We offer Full-Time, Part-Time, Baylor, and Per Diem hours with excellent salary and benefit packages. While hospice experience is beneficial, we provide training for the right individuals. Strong clinical support and educational resources are also available.

Primary Duties and Responsibilities

  • Completes an initial assessment of patient and family to determine any immediate needs and brings them to the Interdisciplinary Team (IDG) immediately to address.
  • Completes the HIS within two days of the election of hospice and again at discharge, in accordance with the hospice CoP’s (Conditions of Participation).
  • Completes nursing comprehensive assessment on admission and election of hospice.
  • Cohorts with the IDG to ensure that all disciplines complete their assessments within five days of the election of hospice, in accordance with the hospice CoP’s.
  • Develops a care plan with the support of the IDG that establishes goals based on the client’s diagnosis, physical, emotional, and spiritual needs.
  • Prepares clinical notes and communicates with the primary physician/ hospice medical director every fourteen days.
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition.
  • Obtains/receives physicians orders as required.
  • Communicates with the entire interdisciplinary team to coordinate/ update the client’s plan of care.
  • Safeguards all required documentation is done and submitted on time in compliance with the hospice CoP’s.

Qualifications, Skills, And Competencies

  • Graduate of an accredited school of nursing.
  • 1-2 years of recent patient centered care/nursing experience.
  • Current state licensure, CPR certification, and a valid driver’s license.
  • Bachelor's degree in nursing with one year of hospice care experience preferred.
  • Strong observation, communication, and problem-solving skills.
  • Physical capability to lift patients and equipment, with considerable walking, standing, reaching, bending, and kneeling.

Physical Requirements

  • Prolonged or considerable walking or standing.
  • Ability to lift, position, or transfer patients (up to 50 lbs.).
  • Frequently reaching, stooping, bending, kneeling, or crouching.
  • Visual acuity and hearing to perform required nursing skills.

Supervisory Responsibilities

  • Supervise and coordinate care with LPNs and LNAs.
