Responsibilities
- Perform patient assessments and develop care plans.
- Ensure appropriate utilization of services and maintain productivity standards.
- Complete discharge assessments and documentation promptly.
- Use age-appropriate teaching methods for patients and families.
- Manage cases effectively, ensuring interdisciplinary communication and adherence to hospice regulations.
- Maintain a safe environment and demonstrate knowledge of pharmacology and medication management.
- Attend monthly staff meetings and participate in performance improvement programs.
- Assist in growing agency business through orientation and cross-training.
Required Skills & Experience
- Two years of experience as an RN in a home hospice environment.
- Experience using an EMR, with excellent critical thinking and communication skills.
- Valid New Jersey driver’s license and BLS certification.
Preferred Skills & Experience
- Three years of RN experience.
Schedule
- Days per week: 5
- Shift: 8a-4p
- Every other weekend with comp days during the week
- Float to different counties according to need
