This role offers a flexible schedule in a setting with fewer distractions, allowing you to do what you do best.

Visiting Health Services of New Jersey (VHSNJ), a partnership between Hackensack Meridian Health and St. Joseph's Health, provides a comprehensive continuum of home health care and hospice services to patients and families in Passaic, Bergen, and parts of Morris counties.

Responsibilities

  • Provide professional nursing care to patients in their home through assessment, development, implementation, and evaluation of home nursing care needs on an individual and continuous basis.
  • Ensure compliance with physician orders in a timely manner, including visit frequency.
  • Document care to reflect compliance with physician’s orders and progression toward goals.
  • Follow policy, procedure, and standards of care for assessment, planning, intervention, and evaluation of patient care.
  • Provide holistic care to patients and their significant others.
  • Ensure safe patient care and demonstrate competency in assigned treatments and procedures.
  • Utilize the interdisciplinary team and document all activities accurately.
  • Apply clinical judgments and critical thinking that reflect professional standards of care and codes of ethics.
  • Identify appropriate payors and remain in compliance with authorizations for self and other disciplines.
  • Maintain visit frequency within authorization range and use secure messaging for insurance communications.
  • Complete nursing assessments, medication reconciliation, and collaborate with physicians on the Plan of Care.
  • Update care plans within two weeks of changes in patient status.
  • Implement teaching strategies to meet established goals on the plan of care.
  • Report patient care problems to management or physicians in a timely manner.
  • Act as a liaison among physicians, managers, and other healthcare team members to coordinate services.
  • Communicate pertinent patient information and Plan of Care during transfers or time-off.
  • Complete routine assignments accurately, consistently, and independently in a timely manner.
  • Meet productivity requirements consistently.
  • Act as a clinical supervisor for para-professional home health aide staff, including supervisory visits and evaluations.
  • Maintain awareness of updated standards and incorporate them into nursing practice.
  • Demonstrate flexibility with job responsibilities and adhere to organizational competencies and standards of behavior.
  • Perform physical tasks such as lifting a minimum of 50 lbs., pushing/pulling 50 lbs., and standing for a minimum of 8 hours a day.

Requirements

  • Associate's Degree in Nursing or Diploma Certificate in Nursing.
  • Minimum of six (6) months of RN experience or at least 1 year of LPN experience, preferably in home care or hospice.
  • NJ State Professional Registered Nurse License.
  • AHA Basic Health Care Life Support HCP Certification.

Preferred qualifications include a BSN, IV Certification, and PC experience/skills.

Pay

Minimum starting rate of $94,161.60 annually. The posted rate is a reasonable good faith estimate and may vary based on factors such as labor market data, experience, education, certifications, skills, geographic location, internal equity, and budget considerations.
