About the role
The Transplant Coordinator Nurse is responsible for coordinating comprehensive care for transplant patients, collaborating with patients, caregivers, and healthcare teams to develop and implement tailored care plans. They assess patient data, interpret information based on age-specific needs, and provide ongoing education to patients and families.
Responsibilities
- Collaborate with patients, caregivers, and healthcare teams to develop and implement tailored care plans
- Absess patient data and interpret information based on age-specific needs
- Provide ongoing education to patients and families
- Engage in leadership activities such as community involvement, committee participation, and quality improvement efforts, guided by nursing standards and ethical guidelines
Qualifications
- Baccalaureate nursing degree from an accredited program
- At least 1 year of clinical RN experience (3 years preferred)
- Flexible working hours
- Demonstrate leadership
- Possess strong communication, critical thinking, and computer skills
- Certification as a Clinical Transplant Coordinator or Nurse, or Oncology Certification, obtained within three years of hire
- Valid RN licensure and BLS certification required
- Adaptability, autonomous and team work capabilities, and ongoing certification maintenance
