Registered Nurse – Transitional Care & Emergency Department

Role: Clinical Nurse in the Transitional Care Unit and Emergency Department. Responsibilities: Provide evidence-based nursing care, conduct comprehensive patient assessments, administer medications, monitor complex conditions, and coordinate care transitions. Support emergency department flow as needed, educate patients and families, document in the electronic health record (EHR), and collaborate with physicians and interdisciplinary teams. Uphold safety, quality, and patient experience standards in a learning-focused environment. Qualifications and Skills: Strong clinical assessment skills, ability to manage complex conditions, effective communication, teamwork, and documentation proficiency. Knowledge of care transition processes and emergency care environment preferred.
