Registered Nurse Skilled Nursing: Mahoning Valley

About the role

The Skilled Nursing Registered Nurse (RN) provides comprehensive, patient-centered skilled nursing care in the home setting. The RN visits multiple patients per day and collaborates with physicians, therapists, caregivers, and community resources to promote optimal patient outcomes while ensuring care is delivered safely, effectively, and in accordance with agency, state, and federal standards.

Responsibilities

  • Provide skilled nursing care that meets professional, regulatory, and organizational standards.
  • Complete comprehensive patient assessments and ongoing reassessments to identify patient needs and guide care planning.
  • Develop, implement, evaluate, and revise individualized plans of care in collaboration with physicians, patients, families, and the interdisciplinary team.
  • Carry out physician orders and coordinate care with therapy services, community agencies, and other healthcare providers.
  • Utilize the nursing process, critical thinking, and sound clinical judgment to make independent nursing decisions.
  • Educate patients, families, and caregivers regarding disease management, medications, treatments, equipment, and health promotion.
  • Maintain accurate, timely, and thorough clinical documentation and communicate patient status changes and care needs with physicians and other members of the healthcare team.
  • Participate in the on-call rotation to ensure patient care coverage.
  • Participate in Quality Assessment and Performance Improvement (QAPI) activities, as well as required staff meetings, educational programs, and training.
  • Perform other duties as assigned.

Requirements

  • Bachelor of Science in Nursing (BSN) preferred or must be obtained within five (5) years of hire.
  • Licensed and currently registered as a professional nurse in the State of Ohio.
  • BLS Certification from the American Heart Association (AHA) required.
  • Minimum one (1) year of in-hospital or home care nursing experience required.
  • Experience in collaborating with interdisciplinary healthcare teams preferred.
  • Proficiency with Microsoft Office (Outlook, Word, and Excel).
  • Experience using an electronic medical record (EMR) is preferred.
  • Strong organizational, communication, critical thinking, and time management skills.
