About the role
The Skilled Nursing Registered Nurse (RN) provides comprehensive, patient-centered skilled nursing care in a home setting.
Responsibilities
- Perform thorough patient assessments and develop individualized care plans in collaboration with physicians, patients, families, and the healthcare team.
- Execute physician orders, coordinate care with therapists and community resources, and utilize clinical judgment for independent decision-making.
- Educate patients and caregivers on disease management, medications, treatments, and health promotion.
- Maintain accurate documentation, communicate patient status changes, and participate in quality improvement activities.
- Join on-call rotations to ensure continuous patient coverage.
- Engage in staff meetings, training, and other assigned duties.
Qualifications
- Current RN license in Ohio and BLS certification from AHA.
- Minimum one year of home care or hospital nursing experience.
- Strong organizational, communication, critical thinking, and time management skills.
- Proficiency with Microsoft Office; EMR experience preferred.
- Bachelor of Science in Nursing (BSN) preferred or to be obtained within five years.
Schedule
Part-time, onsite role with working hours from 0800-1530 or 0900-1630 and a FTE of 0.70.
