Registered Nurse - Skilled Nursing

About the role

A Skilled Nursing Registered Nurse (RN) providing patient-centered care in a home setting.

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans in collaboration with healthcare teams.
• Implement physician orders and coordinate with therapy services, community agencies, and caregivers.
• Utilize clinical judgment and critical thinking to make independent decisions.
• Educate patients, families, and caregivers on disease management, treatments, and health promotion.
• Maintain accurate documentation and communicate patient needs to the healthcare team.
• Participate in on-call coverage, quality improvement activities, and staff training.

Requirements

• Current Ohio RN license and BLS certification from AHA.
• Minimum one year of home care or hospital nursing experience.
• Proficiency with Microsoft Office; EMR experience preferred.
• Strong organizational, communication, critical thinking, and time management skills.

Qualifications

• BSN preferred or to be obtained within five years.

Schedule

Part-time role, onsite.
