About the Role

Provide patient-centered nursing care in patients' homes, coordinate care plans with healthcare professionals, and ensure quality and compliance with regulations.

Responsibilities

• Develops, implements, and revises individualized care plans in collaboration with physicians and team members.
• Supervises home health aides and nurses, providing instruction and ensuring adherence to standards.
• Performs comprehensive assessments, completes accurate documentation, and manages patient visits.
• Communicates effectively with patients, families, and healthcare team to ensure quality care and safety.
• Participates in performance improvement, survey readiness, and team activities, including on-call rotations.

Requirements

• Current unrestricted RN license in the practice state.
• Valid driver’s license, vehicle insurance, and access to reliable transportation.

Qualifications & Skills

• Preferred: CPR certification.
• Preferred: 1+ years of home health experience.
• Strong communication and organizational skills.
• Ability to work independently.
