About the role

Registered Nurse in Home Health, responsible for providing and supervising nursing care in patients' homes, coordinating care plans with healthcare professionals, and ensuring compliance with regulations.

Responsibilities

• Developing, implementing, and revising patient care plans in collaboration with physicians and team members
• Supervising care provided by aides and licensed nurses, offering instruction, and performing supervisory visits
• Delivering high-quality clinical services, completing assessments (OASIS), and documenting visits accurately
• Ensuring patient eligibility, communicating effectively with team members, patients, and families, and participating in quality improvement activities
• Supporting team functions, mentoring new staff, and participating in on-call rotations

Qualifications

• Current, unrestricted RN license in the state of practice
• Valid driver's license, vehicle insurance, and reliable transportation

Skills

• CPR certification
• 1+ years of home health experience
• Strong communication and organizational skills
• Ability to work independently
