About the role
Provide direct nursing care to patients in the long term care setting in accordance with established policies, procedures and protocols of the healthcare organization. Implement and monitor patient care plans. Monitor, record and communicate patient condition as appropriate. Serve as a primary coordinator of all disciplines for well-coordinated patient care. Note and carry out physician and nursing orders. Assure resident safety by identifying safety hazards and emergency situations and initiate corrective action as appropriate. Assess and coordinate patient's discharge planning needs with members of the healthcare team.
Responsibilities
- Provide direct nursing care to patients in the long term care setting in accordance with established policies, procedures and protocols of the healthcare organization.

- Maintain accurate and timely documentation of patient care activities.
- Communicate patient condition as appropriate to interdisciplinary team members.
- Note and carry out physician and nursing orders.
- Assure resident safety by identifying safety hazards and emergency situations and initiating corrective action as appropriate.
- Coordinate discharge planning needs with members of the healthcare team.
Requirements
- Licensure / Certification / Registration: BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
- Registered Nurse obtained prior to hire date or job transfer date required.
- Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
Qualifications
- Experience: Experience in long-term care setting preferred.
- Skills: Strong communication and interpersonal skills, ability to work independently and as part of a team, strong organizational skills.

Pay
$46.00 - $48.00 per hour.
