Registered Nurse - Senior Care Coordinator - Full-Time - Day Shift

About the Role

The Care Coordinator RN Sr. assesses patient needs, coordinates care across the continuum, and supports discharge planning for complex cases during hospitalization. The role focuses on delivering patient-centered care, optimizing resource utilization, and ensuring effective transition from hospital to the next care setting.

Responsibilities

• Conducts psychosocial and clinical assessments.
• Collaborates with physicians and care teams.
• Manages discharge planning and ensures timely care progression.
• Acts as a resource for team members and mentors new staff.
• Participates in interdisciplinary rounds and patient education.
• Documents care activities accurately.
• Identifies barriers to discharge and facilitates post-acute referrals.

Requirements

• Associate's or Bachelor's degree in Nursing.
• RN licensure (single state or multi-state).
• BLS certification.
• Minimum of 3 years' acute care experience plus 2 years in hospital-based case management.
