Registered Nurse (RN) - Care Coordinator - Full-Time Day Shift

Role: RN Care Coordinator responsible for assessing, planning, and coordinating patient care across the continuum to facilitate smooth transitions and discharge planning. Key Responsibilities: Conduct psychosocial and functional assessments, collaborate with care teams and families, manage discharge planning, and address barriers to timely discharge. Facilitate interdisciplinary rounds, ensure comprehensive documentation, and support post-acute referrals. Proactively identify delays in care and work to resolve them efficiently. Participate in patient/family education and advocate for patient needs while respecting cultural and religious considerations. Qualifications & Skills: Must have an Associate's Degree in Nursing, a valid RN license, and at least 1 year of acute care nursing experience. Required certifications include RN licensure and BLS. Candidates should possess excellent communication, assessment, organizational, and problem-solving skills, with the ability to work under stress, handle change, and collaborate effectively with healthcare teams and patients. Knowledge of regulations, community resources, and discharge processes is essential. Additional Duties: Support professional development, serve as a mentor, and ensure compliance with policies and standards.
