Registered Nurse - Care Coordinator - Full-Time Day Shift

About the Role

As an RN Care Coordinator, you will assess, plan, and coordinate patient care during transitions, ensuring timely discharge and efficient resource utilization.

Responsibilities

• Assess psychosocial, functional, and medical needs of patients.
• Collaborate with care teams, patients, and families to develop and implement care plans.
• Manage discharge planning, address barriers, and facilitate post-acute care referrals.
• Monitor care progression, resolve delays, and ensure efficient care delivery.
• Document assessments, care plans, and interactions accurately and timely.
• Support professional development and mentor others as needed.

Requirements

• Associate's Degree in Nursing.
• Active Georgia RN License (single or multi-state).
• Minimum 1 year of acute care nursing experience.
• Certifications: BLS (Instructor preferred).
