Responsibilities
- Implements and monitors patient care plans
- Maintains accurate and timely documentation of patient condition
- Serves as a primary coordinator of all disciplines for well-coordinated patient care
- Notes and carries out physician and nursing orders
- Assesses and coordinates patient's discharge planning needs with members of the healthcare team
- Performs initial, re-assessment, and focused assessments based on patient condition, setting, and population
- Identifies problems and develops measurable and achievable short- and long-range goals in collaboration with the patient and family
- Establishes daily goals with the patient and family, formulates a plan of care, and transitions or discharges patients
- Ensures patient safety, administers medications, provides ordered treatments, educates patients and families, and documents care in the patient’s record
- Evaluates both the patient’s status and the effectiveness of the nursing care and modifies the care plan as needed
- Carefully coordinates actions initiated by nurses with patients, families, and/or members of their healthcare team to manage and correct the sequence, timing, and/or effectiveness of patient care from admission to discharge
- Delegates tasks, supervises staff, prioritizes care, and ensures continuity of care
Requirements
- Licenses/Certifications/Registrations: Valid current Arizona Nursing License or compact state license and ability to become Arizona Licensed in 30 days. Basic Life Support (BLS) - American Heart Association, Advanced Cardiac Life Support (ACLS) - American Heart Association. Certification Preferred: Pediatric Advanced Life Support (PALS), Neonatal Resuscitation Program (NRP) or others as required by individual department.
- Education: Required: Graduation from an accredited School of Nursing. Preferred: Bachelors of Science in Nursing
- Work Experience: Required: 1 year of experience as a licensed RN
