Responsibilities

• Provide home-based visits to clients to review plan of care and update as needed
• Educate clients on their plan of care
• Coach family members on the clients plan of care and home safety
• Communicate client changes and provide an updated plan of care to the office team

Requirements

• Graduate of an accredited nursing program and active RN license in CA
• CPR certification, Criminal Background Check, Drug Screen
• Knowledge of state and federal regulations for PCSS
• Clinical proficiency in nursing specialty, strong communication skills and compassion
