Registered Nurse (RN) - Skilled Nursing Facility (SNF) - Long Term Care

Responsibilities

• Providing compassionate, resident-centered care and emotional support to patients, families, and caregivers.
• Developing, implementing, and evaluating personalized care plans based on comprehensive assessments.
• Collaborating with interdisciplinary teams to coordinate patient care, transfers, discharges, and referrals.
• Delegating tasks to ancillary staff and performing supervisory duties as needed.

Qualifications
