Responsibilities
- Conduct comprehensive assessments of patients and families to determine home care needs.
- Provide thorough physical assessments and medical histories.
- Continuously reassess patient needs and adjust care plans accordingly.
- Collaborate with physicians to develop personalized care plans.
- Involve patients and families in the planning process.
- Implement preventive and rehabilitative nursing procedures.
- Administer medications and treatments as prescribed.
- Offer counseling and health education to patients and families.
- Identify discharge planning needs and implement plans prior to discharge.
- Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.
Requirements
- Current licensure in the State of Virginia.
- CPR certification and a valid driver’s license.
- Minimum of one year of recent acute or long-term care experience (preferred).
- Bachelor’s degree preferred, with at least one year of experience in home care.
- Responsibility for supervising Home Health Aides and Personal Care Aides.
- Strong observation, verbal, and written communication skills.
- Excellent problem-solving abilities and basic math skills.
