Responsibilities

  • Conduct comprehensive assessments of patients and families to determine home care needs.
  • Provide thorough physical assessments and medical histories.
  • Continuously reassess patient needs and adjust care plans accordingly.
  • Collaborate with physicians to develop personalized care plans.
  • Involve patients and families in the planning process.
  • Implement preventive and rehabilitative nursing procedures.
  • Administer medications and treatments as prescribed.
  • Offer counseling and health education to patients and families.
  • Identify discharge planning needs and implement plans prior to discharge.
  • Perform peripheral short intravenous line establishment and administer IV medications under physician orders and with demonstrated competency.

Requirements

  • Current licensure in the State of Virginia.
  • CPR certification and a valid driver’s license.
  • Minimum of one year of recent acute or long-term care experience (preferred).
  • Bachelor’s degree preferred, with at least one year of experience in home care.
  • Responsibility for supervising Home Health Aides and Personal Care Aides.
  • Strong observation, verbal, and written communication skills.
  • Excellent problem-solving abilities and basic math skills.
