Registered Nurse - RN - Senior LIFE Johnstown

About the role

Our RNs work as an extension of our Clinic Team but with the autonomy and flexibility of providing that care in the home and community settings. We serve the senior population of Johnstown and a limited surrounding area so you won’t spend all of your time in a car. You will be doing what you do best in providing the care to enable our participants to live their best lives exactly where they most want to be - at home!

Responsibilities

• Provide high-quality, compassionate care to seniors in their homes and communities.
• Assess and document patient needs and progress.
• Collaborate with other healthcare providers to ensure comprehensive care plans.
• Ensure compliance with all regulatory requirements and safety protocols.
• Develop and maintain positive relationships with participants, their families, and fellow team members.

Requirements

• Current Registered Nurse license in the state of employment.
• Experience in home health care or related field preferred.
• Strong communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Valid driver’s license and reliable transportation (if applicable).

Qualifications

• Graduation from an accredited nursing program.
• Passing scores on the NCLEX-RN exam.
• Basic computer skills for electronic health records (EHR).

• Strong organizational and problem-solving skills.
• Knowledge of home health care practices and regulations.
