Registered Nurse: RN : Per Diem

About the Role

Alvita Care is seeking experienced, CT-licensed Registered Nurses (RNs) to provide private duty nursing services in Western Connecticut (Fairfield, New Haven, Litchfield County). Ideal candidates are reliable, compassionate, and willing to deliver consistent, one-on-one care in the home environment.

• Flexible hours with shifts ranging from 6, 8, to 12 hours
• Day and night shifts available
• Opportunities to make skilled nursing visits
• Work part-time, per diem, or around your current schedule
• Shifts assigned for 1:1 care in the home

Responsibilities

• Assess client needs to develop a plan of care
• Consult with management on complex or unique cases
• Collaborate with clinical, medical, and multidisciplinary team members to ensure care plan adherence
• Monitor and assess patient condition and the effectiveness of the care plan
• Oversee and supervise care provided by Licensed Practical Nurses and paraprofessionals
• Manage, plan, coordinate, and execute safe patient discharge from service
• Ensure physician’s orders, discharge summaries, and communications adhere to company and regulatory requirements
• Communicate and consult with patient’s physician as needed
• Manage and coordinate all aspects of assigned caseload
• Assist patients with activities of daily living
• Document all services provided in the internal system accurately
• Transport and utilize medical equipment to and from clients’ homes or facilities
• Travel to patients’ homes and/or other facilities
• Perform other related duties and participate in special projects as assigned

Requirements

• CT RN licensure
• BLS, ACLS, or CPR certification
• Minimum of 6 months to 1 year of demonstrated experience as an RN
• Ability to put patients at ease and explain physician’s instructions clearly
• Excellent multitasking and organizational skills
• Experience working with diverse racial, ethnic, and socioeconomic backgrounds
• Knowledge of medical terminology
• Ability to work both independently and as a team player
• Valid driver’s license or non-driver photo ID card (drivers preferred)
