Registered Nurse (RN) - Patient Navigator

About the Role

Provides patient referrals, education, and guidance for orthopedic and cardiac DRGs, ensuring care aligns with evidence-based and regulatory standards.

Responsibilities

• Coordinates referrals from physicians and healthcare facilities.
• Delivers comprehensive patient education about illnesses and treatments.
• Guides patients through the care continuum.
• Completes required competencies and performs related duties as assigned.

Requirements

• Associate’s Degree in Nursing (Bachelor’s preferred).
• Minimum of 3 years clinical experience in the specialty area.
• Current Texas RN license; specialty certification preferred.
• Active healthcare provider BLS certification (AHA or ARC), with AHA Healthcare Provider BLS within 60 days.
