RN Patient Navigator in Critical Care at a medical center.
Responsibilities
- Serve as a central contact for patients and families, ensuring understanding of diagnoses and treatment plans.
- Facilitate smooth care transitions from ICU to other units or home, and address barriers such as financial or social issues to ensure timely access to services.
- Provide follow-up education to reduce readmissions, offer emotional and psychosocial support, and collaborate with interdisciplinary teams to optimize care workflows.
Qualifications
- Active Registered Nurse license from the state.
- BLS certification preferred.
- Diploma or equivalent in nursing.
- Preference for candidates with 5+ years of critical care experience, especially in ICU settings.
