Comfort Home Care is seeking a part‑time weekend Registered Nurse (RN) or Licensed Practical Nurse (LPN) to serve psychiatric and home‑health patients in the Fitchburg/Leominster area. Responsibilities

• Assess and Adapt: Evaluate each patient’s current condition, medical needs, and overall well‑being at the start of every visit.
• Create and Modify Care Plans: Shape patients’ treatment plans based on their needs and adjust care as conditions change.
• Teach and Support: Provide medication guidance for psychiatric patients, wound‑care education for post‑op patients, and other instruction to help patients stay independent at home.
• Communicate and Coordinate: Keep providers, families, and care teams informed of changes to ensure timely, appropriate care.
• Document and Reflect: Submit accurate visit notes from home for compliance and continuity of care.

Requirements

• Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in Massachusetts.
• Minimum one year of nursing experience (home health or psychiatric experience preferred).
• A drive to provide compassionate, patient‑centered care.
• Reliable transportation for patient visits.
