What You'll Do
- Cook up the Resident Assessment Instrument (RAI) process to ensure accurate and timely completion of resident assessments
- Ensure compliance with Medicare, Medicaid, OBRA, and other payer program requirements
- Ensure MDS assessments accurately reflect each resident's physical, mental, and psychosocial status
- Verify documentation supports services provided and reimbursement accuracy
- Support interdisciplinary collaboration for accurate care planning, assessment completion, and ongoing case management coordination
- Collaborate with clinical and administrative teams regarding admissions, transitions of care, and payer communication related to skilled nursing services
- Monitor assessment schedules and ensure regulatory timelines are met
- Aid in quality improvement initiatives and survey readiness efforts
Required Qualifications
- Graduate of an accredited school of nursing with a current RN license in the state of employment
- MDS 3.0 and case management experience in a skilled nursing or post-acute care setting required
- Ability to work independently while collaborating effectively with interdisciplinary teams
- Strong organizational, communication, and critical thinking skills
- Working knowledge of Microsoft Office applications
Preferred Qualifications
- Previous long-term care or skilled nursing experience
- RAC-CT certification or willingness to obtain within six months of hire and maintain certification requirements
